After doing Examine on Locked Account I don't see any details

I am having issue in trying to resolve why we are keep getting an AD account Locked after user changed their password last week. Normally this gets resolved but in this case it is related to a password that is needed when he loads our SharePoint Home page only. We have an application that uses SharePoint that he uses for a month every 3 months only. We have deleted the browsing history in both IE 11 and Latest version of Chrome(preferred browser) to remove any saved passwords which has worked before when this issue occurs but not in this case.


I have noticed that we are not getting the expected log details when we examine the respective AD account as per attached file. Note that the AD account used to run this is my DA account that should have required access rights as well.


Is someone able to assist me in what I need to do to resolve this please.
